Job summary

NAFI Connecticut, Inc. is seeking a dynamic, compassionate part-time Case Aide for our Family wraparound program.

The aide will support families at convenient locations, often the client home or community setting, and assist case managers and clinicians in delivering services. Responsibilities include guiding families toward service plan goals, coordinating transportation to meetings, and maintaining documentation in Evolv and CNNX.

Qualifications

  • High school diploma required; associate's degree preferred.
  • Bi-lingual (English/Spanish) applicants strongly encouraged to apply.
  • Must hold a valid driver's license and have own vehicle.

Job description

NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. We believe that people can change, and we work to empower each individual in their own care.

NAFI NY is looking for dynamic, compassionate, part-time Case Aide for our Family wraparound program. The case aide will support families by providing services to address concrete needs based on the recommendations of the case managers and clinician. The case aide will provide services to families at a location convenient to them, most often client home or community setting.

Responsibilities Include:

  • Provide support and guidance to the program families and youth around service plan goals
  • Provide support to case managers and clinicians as needed to support service delivery to families
  • Team approach to connect families with service providers to support families achieving goals
  • Provide transportation for families as needed to support attendance to meetings and appointments
  • Facilitate group and respite activities as needed
  • Conduct collateral contacts to obtain required documentation for youth and families in the program
  • Maintained required documentation in Evolv and CNNX

Must hold a valid driver's license, have own vehicle and willingness to travel periodically to offices in Orange County, Westchester County and other locations as required to fulfill job responsibilities

Desired Qualifications:

  • High school diploma required, associate's degree preferred
  • Two to three years related experience and/or training
  • Experience with youth who experienced trauma or involvement with the child welfare and/or juvenile justice system preferred
  • Bi-lingual (English / Spanish) applicants strongly encouraged to apply

Hourly Rate: $16.00 - $18.00 per hour
