ProMedica is seeking a Transition Coordinator at the Flower Rehab Center to manage referrals and coordinate admissions for patient transitions. The role involves medical record review and collaboration with physicians, nursing, therapists, leadership, and clinical documentation specialists.

The coordinator will work with PPS and Case Managers to coordinate precertification, recertification, authorizations, appeals, length of stay, and resource use across facilities.

Qualifications

• Minimum of 2 years clinical experience in a patient care setting.
• Experience with utilization review, marketing, care transitions, customer service, and/or inpatient rehab.
• Must be able to commute between facilities with reliable transportation.

Responsabilités

• Manage referrals and coordinate admissions for patient transitions to and from Flower Rehab Center.
• Review medical records and interact with physicians, nursing, therapists, leadership, coding, and clinical documentation specialists.
• Collaborate with PPS Coordinator, Case Managers, Transition Care Managers, and Nursing/ Therapy Leadership to coordinate insurance precertification, recertification, authorizations and appeals, length of stay, and resource utilization.
