Job summary

ProMedica is seeking a Transition Coordinator at the Flower Rehab Center to manage referrals and coordinate admissions for patient transitions. The role involves medical record review and collaboration with physicians, nursing, therapists, leadership, and clinical documentation specialists.

The coordinator will work with PPS and Case Managers to coordinate precertification, recertification, authorizations, appeals, length of stay, and resource use across facilities.

Qualifications

• Minimum of 2 years clinical experience in a patient care setting.
• Experience with utilization review, marketing, care transitions, customer service, and/or inpatient rehab.
• Must be able to commute between facilities with reliable transportation.

Responsibilities

This role actively collaborates with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing and Therapy Leadership to coordinate the timely management of insurance precertification, recertification, authorizations and appeals, length of stay, and resource utilization.The above summary is intended to describe the general nature and level of work performed by the positions. It should not be considered exhaustive.REQUIREMENTSEducation: Post High School clinical training equivalent to a certification or licensureSkills: Strong verbal and non-verbal communication skills both at patient and healthcare professional levels. Ability to work in a team environment. Ability to input, retrieve and analyze relevant EMR data. Mid-level Microsoft Word and Excel skills. Must be able to commute between facilities with reliable transportation. Must be able to understand directions, communicate, and respond to inquiries with effective interpersonal skills. Must have good organizational, analytical, writing, and interpersonal skills. Must be able to input and retrieve information from electronic devices and must be able to operate copier, electronic device(s), printer, and scanner.Years of Experience: Minimum of 2 years clinical experience in a patient care setting.
