ProMedica is seeking a Transition Coordinator at the Flower Rehab Center to manage referrals and admissions during patient transitions. You will collaborate with physicians, nurses, therapists, and leadership to ensure timely insurance precertification, approvals, and optimal resource use.

The role requires clinical experience and strong communication, with duties across medical record review, interdepartmental coordination, and patient/family communication.

Qualifikationen

• Post High School clinical training or licensure required.
• 2+ years clinical experience in patient care setting preferred.
• Experience with utilization review, care transitions, or inpatient rehab a plus.

Aufgaben

• Manage referrals and coordinate admissions for transition of patients to and from Flower Rehab Center.
• Review medical records and interact with physicians, nursing, therapists, leadership, and care teams.
• Coordinate insurance precertification, recertification, authorizations and appeals; manage length of stay and resource use.
