ProMedica Monroe Physicians is seeking a Transition Coordinator to manage referrals and coordinate admissions for patient transitions at the Flower Rehab Center. You will perform medical record reviews and liaise with physicians, nursing, therapists, leadership, and staff to ensure accurate and timely processes.

You will collaborate with the FRC PPS Coordinator, Case Managers, Transition Care Managers and Nursing/ Therapy Leadership to secure precertifications, recertifications, authorizations

Qualifications

• Post high school clinical training or licensure is required.
• Minimum 2 years of clinical experience in a patient care setting; experience with utilization review, marketing, care transitions, patient services and/or inpatient rehab preferred.
• CPR certification and licensure as indicated by education obtained.

Responsabilités

• Manage referrals and coordinate admissions for transitions to Flower Rehab Center.
• Perform medical record review and liaise with physicians, nursing, therapists, leadership, coding and clinical documentation specialists, patients, caregivers and the community.
• Coordinate precertification, recertification, authorizations and appeals, length of stay and resource utilization; ensure timely processes.
• Collaborate with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing and Therapy Leadership to support smooth patient transitions.
