Job summary

The Transition Coordinator is a key role at ProMedica Monroe Physicians, responsible for managing referrals and coordinating admissions for patients transitioning to and from the Flower Rehab Center. You will review medical records, interact with physicians, nursing, therapists, and care teams, and help secure timely insurance precertifications and authorizations.

You will collaborate with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing Leadership to optimize length

Job Summary:

The Transition Coordinator is responsible for managing referrals and coordinating admissions for the transition of patients to and from the Flower Rehab Center. This role involves medical record review and interaction with internal and external physicians, nursing, therapists, leadership, coding and clinical documentation specialists, patients, caregivers, and the community. This role actively collaborates with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing and Therapy Leadership to coordinate the timely management of insurance precertification , recertification, authorizations and appeals, length of stay, and resource utilization . REQUIREMENTS

• Education: Post High School clinical training equivalent to a certification or licensure
• Skills: Strong verbal and non-verbal communication skills both at patient and healthcare professional levels. Ability to work in a team environment. Ability to input, retrieve and analyze relevant EMR data. Mid-level Microsoft Word and Excel skills. Must be able to commute between facilities with reliable transportation. Must be able to understand directions, communicate, and respond to inquiries with effective interpersonal skills. Must have good organizational, analytical, writing, and interpersonal skills. Must be able to input and retrieve information from electronic devices and must be able to operate copier, electronic device(s), printer, and scanner.
• Years of Experience: Minimum of 2 years clinical experience in a patient care setting. Previous experience with utilization review, marketing, care transitions, customer service, and/or inpatient rehab.
• License: As indicated per education obtained.
• Certification: CPR and as indicated per education obtained.

PREFERRED REQUIREMENTS

• Education: Associate degree in a clinical field.
• Skills: Ability to work with a variety of internal/external customers and healthcare providers effectively and diplomatically.
• Years of Experience: 3-5 years clinical experience in a patient care setting.
