Job summary

The Transition Coordinator is a key role at ProMedica Monroe Physicians, responsible for managing referrals and coordinating admissions for patients transitioning to and from the Flower Rehab Center. You will review medical records, interact with physicians, nursing, therapists, and care teams, and help secure timely insurance precertifications and authorizations.

You will collaborate with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing Leadership to optimize length

Qualifications

• Certification or licensure per education.
• Minimum of 2 years clinical experience in patient care.
• CPR certification.
• Ability to commute between facilities.
• Strong organizational and interpersonal skills.

Responsibilities

• Manage referrals and admissions transitions.
• Review medical records and coordinate with physicians, nursing and therapy staff.
• Coordinate insurance precertification, recertification, authorizations and appeals.
• Collaborate with PPS Coordinator and Case Managers to optimize LOS.
• Ensure appropriate resource utilization and effective patient communication.
