Job summary

VITAS Healthcare seeks a Palliative Care Registered Nurse (RN) to join a comprehensive Palliative Care Team providing individualized, coordinated care in the community. The role acts as Case Manager, coordinates patient care for an assigned caseload, and supports transitions of care between hospital, primary and specialty services.

The position emphasizes education, emotional support, and collaborative care planning to improve patient outcomes and adherence to treatment plans.

Qualifications

• Minimum 2 years’ experience as Registered Nurse.
• 1–2 years Case Management experience.
• 1 year of community/home health, rehab, hospice, etc. experience preferred.
• Knowledge of the interdisciplinary team concept.
• Exceptional communication and customer service skills.
• Reliable transportation, current state driver’s license and automobile insurance.
• Bilingual a plus.

Responsibilities

• Acts as Case Manager and coordinates care for assigned caseload.
• Completes initial and ongoing comprehensive assessments of patient and family.
• Ensures referrals to specialists and supports adherence to treatment plan.
• Educates patient and family about disease processes and plan of care.
• Provides emotional support and counseling to patients and families.
• Evaluates recovery and progress post-treatment.
• Encourages advance care planning and directives.
• Builds relationships with other clinicians involved in care.
• Facilitates transitions of care between hospital, primary and specialty care.
• Supports discharge planning and follow-up to improve outcomes.
• Protects patient privacy and confidentiality.
