Job summary

VITAS Healthcare is seeking a Registered Nurse (RN) to join the interdisciplinary team providing holistic care to hospice patients and families confronting terminal illness. The RN coordinates care, conducts ongoing assessments, and implements the interdisciplinary plan of care at the patient’s location.

This role includes education for patients and families, participation in team meetings, support during bereavement, and on-call coverage to meet program needs.

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Coordinates care and implements the interdisciplinary plan of care
• Completes initial and ongoing comprehensive assessments
• Provides education and training to patients, families, caregivers and referral sources
• Participates in patient care meetings and weekly team meetings
• Delivers shift reports and communicates effectively
• Participates in bereavement support and on-call coverage
