Job summary

VITAS Healthcare is seeking a Registered Nurse (RN) to join the hospice interdisciplinary team in delivering compassionate end-of-life care. The RN coordinates complex care plans, conducts ongoing assessments, and ensures adherence to the interdisciplinary plan of care for patients and families facing terminal illness.

The role involves case management, supervision of aides and licensed staff, education for families, and participation in team meetings and quality improvement initiatives.

Qualifications

• Current licensed to practice nursing in the state where the VITAS program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Follows VITAS policies and standards and practices within the RN license scope
• Reports any request to practice outside of license scope
• Supervises nursing tasks delegated to Hospice Aides, LPNs, VNs and Volunteers
• Completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, bereavement care
• Identifies/utilizes resources and referrals (PT, OT, Home Health, Dietary, Pharmacy, equipment)
• Completes documentation at time of event and location of care
• Provides education and training to patients, families, caregivers and referral sources
• Participates in patient care meetings and weekly team meetings
• Provides shift reports through written/verbal communication
• Participates in bereavement support and on-call rotations
• Provides visits/shifts in program coverage area
• Develops and coordinates the plan of care with IDG
• Obtains physician orders as necessary
• Delivers services requiring substantial nursing skill
• Evaluates outcomes of services
• Delegates tasks to Hospice Aides/Volunteers appropriately
• Ensures patient safety factors are assessed in all activities
• Delivers primary nursing care for inpatient and continuous care patients
• Participates in Quality Improvement and QAPI activities
• Engages in professional development and orientation of staff

Education

Nursing license (state where VITAS program is located)

Two years of nursing experience in hospice/home health/community health (last five years)
