Job summary

VITAS Healthcare is seeking a Registered Nurse (RN) to coordinate care for hospice patients and families, ensuring comprehensive assessments and implementation of the interdisciplinary plan of care. The role involves education, collaboration with the IDG, and on-call coverage as needed.

The RN will work within VITAS policies, supervise nursing tasks, and utilize referrals to support patient and family needs, while documenting care events and maintaining strong communication with the care team.

Qualifications

• Currently licensed to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.

Responsibilities

• Case Management and Supervision for Registered Nurse, RN.
• Follows VITAS policies and standards and practices within the scope of the RN license.
• Performs supervision of nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses and Volunteers.
• Completes initial and ongoing comprehensive assessments to palliate end-of-life symptoms.
• Identifies resources and referrals to support patient care.
• Documents events and care locations as required.
• Provides education and training to patients, families and caregivers.
• Participates in patient care meetings and weekly team meetings.
• Maintains effective communication and shift reports.
• Participates in bereavement support and on-call rotation.
