Job summary

VITAS Healthcare in the United States is seeking a Palliative Care Registered Nurse (RN) to support its growing palliative program. The RN acts as a Case Manager coordinating care for a caseload in the community and collaborates with the interdisciplinary team.

The role emphasizes patient and family education, transitions of care, discharge planning, and adherence to treatment plans to improve outcomes.

Qualifications

• Minimum 2 years’ RN experience.
• 1-2 years Case Management experience.
• 1 year of community (home health, rehab, hospice, etc.) experience preferred.
• Knowledge of the interdisciplinary team concept.
• Experience in patient education, planning, and management desired.
• Exceptional communication and customer service skills.
• Reliable transportation, current state driver’s license and automobile insurance.
• Bilingual a plus.

Responsibilities

• Acts as Case Manager and coordinates patient care for assigned caseload.
• Completes initial and ongoing assessments of patients and families.
• Ensures referrals to specialists and adherence to treatment plans.
• Assists patients in accessing services and resources in the community.
• Educates patients and families to enhance understanding of disease and care options.
• Provides emotional support and counseling to patients and families.
• Evaluates recovery and promotes goal setting and advance directives.
• Builds relationships with clinicians involved in care and coordinates transitions of care.
• Facilitates discharge planning and follow-up to improve outcomes.
• Protects patient privacy and safeguards confidentiality.
