Job summary

VITAS Healthcare is seeking a Palliative Care Registered Nurse (RN) to support its growing palliative care program in the community. The selected candidate will be part of a comprehensive Palliative Care Team, coordinating patient care for a caseload and providing individualized, coordinated care to patients and families in the community.

Qualifications

• Minimum 2 years’ experience as Registered Nurse.
• 1-2 years Case Management experience.
• 1 year of community (home health, rehab, hospice, etc.) experience preferred.
• Knowledge of the interdisciplinary team concept.
• Experience in patient education, planning, and management desired.
• Exceptional communication and customer service skills.

Responsibilities

• Completes initial and on-going comprehensive assessments of the patient and family.
• Coordinate patient care for assigned caseload and refer to specialists as needed.
• Assist patients in accessing services and community resources.
• Educate patient and family to enhance understanding of disease and care plan.
• Provide emotional support to patients and families.
• Evaluate recovery and plan follow-up care to improve outcomes.
• Promote patient-centered care and transitions between care settings.
• Build relationships with other clinicians involved in care.
