Job summary

VITAS Healthcare Corporation is seeking a Palliative Care Registered Nurse (RN) to support its growing program in the community. You will act as Case Manager, coordinating patient care, supporting discharge planning, and educating patients and families about disease processes and treatment options.

The role requires a minimum of 2 years as an RN, 1–2 years in case management, and community/home health experience is preferred. BLS certification is required and bilingual skills are a plus.

Qualifications

• Minimum 2 years’ experience as Registered Nurse.
• 1-2 years Case Management experience.
• 1 year of community (home health, rehab, hospice, etc.) experience preferred.
• Knowledge of the interdisciplinary team concept.
• Experience in patient education, planning, and management desired.
• Exceptional communication and customer service skills.
• Reliable transportation, current state driver’s license and automobile insurance.
• Bilingual a plus.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patients and families.
• Coordinate patient care with the interdisciplinary team to manage caseload.
• Educate patients and families to enhance understanding of disease and plan of care.
• Facilitate referrals to community resources and services.
• Promote transitions of care and discharge planning to improve outcomes.
• Protect patient privacy and safeguard confidentiality.
