Job summary

Infinity Hospice Care LLC is seeking a Nurse Practitioner to support palliative care services within the hospice program. You will collaborate with physicians, lead the Interdisciplinary Group, and conduct home visits to assess and manage terminal patient conditions.

The role emphasizes face-to-face encounters, staff education, and coordination with the hospice physician to ensure high-quality end-of-life care for patients and families.

Qualifications

• Ability to establish and maintain effective working relationships with patients, families, staff, and physicians.

Responsibilities

• Collaborate with the IDG/IDT for palliative treatment and plan of care.
• Provide clinical direction and education to staff on palliation and end-of-life care.
• Perform and document home visits and face-to-face encounters as required.
• Communicate changes in patient needs to physicians and the care team.
• Provide medical direction and education to clinical staff and act as a role model.
