Job summary

Infinity Hospice Care LLC is seeking a Nurse Practitioner to support physicians in delivering compassionate end-of-life care. You will lead the Interdisciplinary Group, conduct face-to-face encounters, and provide clinical direction to ensure quality patient outcomes.

Requires NP licensure and an APN background with hospice or home health experience, plus strong communication with families and medical staff.

Qualifications

• Graduate from a Baccalaureate School of Nursing and NP-focused master’s or doctoral program.
• Current RN license and Advanced Practice Nurse licensure; valid driver’s license.
• Two years of APN experience; hospice or home health preferred.
• Knowledge of hospice care, end-of-life care, and related ethics.

Responsibilities

• Collaborates with IDG/IDT to ensure palliative evaluation and treatment.
• Consults on care plans to ensure quality patient and family care.
• Educates patients and families about disease, self-care, and end-of-life care.
• Performs and documents home visits and visits per plan of care.
• Provides medical direction and education to clinical staff.
• Communicates with physicians to optimize patient care outcomes.
• Performs on-call responsibilities and provides on-call services.
