Job summary

Infinity Hospice Care LLC seeks a Nurse Practitioner to support physicians in clinical care and family support within hospice settings. You will conduct face-to-face documentation and home visits as part of palliation and end-of-life care.

You will lead and educate the Interdisciplinary Group/Team, coordinating with the hospice physician to ensure quality patient care and timely communication. A valid RN and APRN license with at least two years in hospice/home health is required.

Qualifications

• Must have strong interpersonal communication with patients, families, and IDG/IDT members (≤140 chars).
• Ability to educate staff and collaborate with physicians for palliation (≤140 chars).
• Experience with home visits and palliative care planning (≤140 chars).

Responsibilities

• Collaborate with IDG/T to ensure palliation and appropriate care.
• Develop and update patient/family care plans with IDG/T.
• Educate patients, families, and staff on disease processes and self-care.
• Perform and document patient/family visits per plan of care.
• Communicate changes in patient needs to physicians and staff.
• Provide clinical direction and consultation for palliation to the care team.
• Complete face-to-face encounters and report results to hospice physician.
