Job summary

Eternal Faith is seeking a Nurse Practitioner for Palliative Care Services to support patients and families in home and community settings. The NP collaborates with physicians and the hospice team to provide clinical direction and direct patient care, including face-to-face visits and documentation.

Requirements include NP degree, active RN and NP licenses, and 2 years of APN experience in hospice or home health.

Qualifications

• NP program graduate (master's or doctoral)
• Active RN license and APRN licensure
• Minimum 2 years APN experience in hospice/home health

Responsibilities

• Collaborates with IDG/IDT to evaluate and plan palliation.
• Performs and documents face-to-face visits per plan of care.
• Educates patients and families about disease, self-care, and end-of-life care.
• Provides clinical direction to the care team and coordinates with physicians.
