Job summary

The Blue-Summit-Hospice-and-Palliative-Car seeks a Nurse Practitioner to provide patient-centered care to terminally ill hospice patients in home settings. You will collaborate with the care team, prepare and adjust treatment plans, and support families throughbereavement.

Responsibilities include independent practice, supervising nursing staff, and coordinating care across settings while maintaining HIPAA-compliant records.

Qualifications

• Graduate from an accredited nursing program with NP certification.
• Licensed as a nurse practitioner in the state, with current active NP license.
• Minimum 2 years NP experience preferred.
• Ability to work independently and manage home-based hospice care.

Responsibilities

• Assess and reassess hospice patient needs regularly.
• Order, perform, supervise and interpret diagnostic tests.
• Develop and manage treatment plans and prescribe medications.
• Educate and counsel patients and families about care at home.
• Supervise nursing staff and coordinate with IDG.
• Maintain patient records and comply with HIPAA.
• Participate in team conferences and on-call coverage.
• Coordinate care for patients in SNF, NF, ICF or MR.
