Job summary

Intermountain Health is seeking a Nurse Practitioner to provide Palliative Care at patients' homes in the Greater Denver Area. You will travel to patient homes, deliver comprehensive palliative services, and collaborate with a team of professionals to ensure exceptional patient experiences.

The role requires an APRN, board certification, and active Colorado license with DEA/ prescriptive authority. Intermountain Health provides benefits and supports credentialing through its PEAK program.

Qualifications

• APRN from an accredited program.
• Board certification required.
• Active Colorado medical license or ability to obtain one.
• Full prescriptive authority and active DEA license or ability to obtain one.
• Must successfully complete Intermountain Health’s credentialing process.

Responsibilities

• Provide palliative care to homebound patients in the Greater Denver Area.
• Travel to patients' homes to deliver care.
• Coordinate with interdisciplinary team to ensure quality patient experience.
