Job summary

Infinity Hospice Care LLC seeks a Nurse Practitioner to assist physicians in overseeing the clinical care of patients and families within hospice settings. You will provide clinical direction to the Interdisciplinary Group/Team, conduct face-to-face documentation, and perform home visits for palliation and management of terminal illness.

You will collaborate with attending physicians, educate patients and families, and participate in ongoing staff training.

Qualifications

• Graduate from a NP-focused Master’s or Doctoral Nursing Program.
• Current state license as a Registered Nurse and as an Advanced Practice Nurse.

Responsibilities

• Collaborates with the Interdisciplinary Group/Team to ensure quality palliative care.
• Provides clinical direction and consultation for palliation and end-of-life care.
• Performs and documents home visits and face-to-face encounters.
• Educates patients, families, and staff on disease processes and care goals.
• Communicates changes in patient needs to physicians and IDG/IDT members.
