Job summary

Impact Health PC is seeking a Palliative Care Nurse Practitioner in Katy, TX. The role offers a flexible Monday–Friday schedule with no nights, weekends, or holidays, alongside a clear earning potential and robust clinical support.

You will assess patients in skilled facilities, develop individualized care plans, and lead goals-of-care discussions with families while collaborating with an interdisciplinary team. Training, benefits, and malpractice coverage are provided.

Qualifications

• Active NP license in Texas.
• Minimum 2 years post-certification experience preferred.
• Palliative care, SNF, hospitalist, ED, or ICU experience preferred.
• Strong communication and interpersonal skills, especially with patients and families.
• Ability to work independently within an interdisciplinary care team.

Responsibilities

• Provide compassionate, patient-centered palliative care using evidence-based practices.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Manage symptoms and provide ongoing follow-up for serious illnesses.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and interdisciplinary care teams.
• Educate patients, families, and facility staff regarding palliative care plans.
• Complete timely and accurate clinical documentation.
• Maintain compliance with facility, state, and federal regulations.
