Impact Health PC in Katy, TX is seeking a Palliative Care Nurse Practitioner to deliver compassionate, patient-centered care for individuals with serious illnesses in skilled nursing facilities. You will assess symptoms, develop treatment plans, and support goals-of-care discussions with patients, families, and care teams.

Qualifications

• Active NP license in Texas.
• Minimum 2 years post-certification experience preferred.
• Palliative care, SNF, hospitalist, ED, or ICU experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and collaboratively as part of an interdisciplinary care team.
