Impact Health PC in Jonestown, PA is seeking a Palliative Care Nurse Practitioner to deliver compassionate, patient-centered care in skilled nursing facilities. You will assess symptoms, create tailored treatment plans, and partner with families and care teams for goals-of-care discussions.

This role emphasizes independent practice with clinical support, documentation, and regulatory compliance to ensure high-quality patient care.

Qualifications

• Active Nurse Practitioner license in PA (required).
• Minimum 2 years post-certification experience preferred.
• Palliative care, SNF, hospitalist, ED, or ICU experience preferred.
• Strong communication and interpersonal skills, especially with patients and families.
• Ability to work independently and as part of an interdisciplinary care team.

Responsabilités

• Provide compassionate, person-centered palliative care using evidence-based practices.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and interdisciplinary care teams.
• Educate patients, families, and facility staff regarding palliative care and treatment plans.
• Complete timely and accurate clinical documentation.
• Maintain compliance with facility, state, and federal regulations.

Formation

Nurse Practitioner license PA

2+ years post-certification experience

Palliative care, SNF, hospitalist, ED, ICU experience
