Job summary

Impact Health PC is seeking a compassionate Nurse Practitioner to provide palliative care within skilled nursing facilities. You will assess symptoms, develop individualized plans, and support goals-of-care discussions with patients, families, and the care team.

You will work with an interdisciplinary group, maintaining independent practice while receiving physician-level consult access and ongoing clinical support.

Qualifications

• Active NP license in MA.
• Minimum 2 years of post-certification experience preferred.
• Palliative care, SNF, hospitalist, ED, or ICU experience preferred.
• Strong communication and interpersonal skills, particularly with patients and families.

Responsibilities

• Provide compassionate, patient-centered palliative care.
• Conduct comprehensive assessments and develop treatment plans.
• Manage symptoms and follow up with patients with serious illnesses.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and interdisciplinary teams.
• Educate patients, families, and staff about palliative care and plans.
