Job summary

Impact Health PC in Lancaster, PA, seeks a Nurse Practitioner to provide compassionate, patient-centered palliative care for seriously ill residents in skilled nursing facilities.

You will assess symptoms, create individualized plans, and support goals-of-care discussions while collaborating with physicians, staff, and families. Independent practice with strong clinical support and ongoing education is offered.

Qualifications

• Active Nurse Practitioner license in PA.
• Minimum 2 years of post-certification experience preferred.
• Palliative care, SNF, hospitalist, or ICU experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Provide compassionate, patient-centered palliative care.
• Conduct comprehensive assessments and develop treatment plans.
• Manage symptoms and follow patients over time.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and care teams.
• Document timely and accurate clinical records.
