Qualifications

• Ohio NP license required.
• Minimum 2 years post-certification experience preferred.
• Palliative care or SNF/hospital settings experience preferred.

Responsibilities

• Provide compassionate, patient-centered palliative care using evidence-based practices.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Manage symptoms and follow-up for patients with serious illnesses.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and interdisciplinary care teams.
• Educate patients and families and complete timely clinical documentation.
