Job summary

CentraCare Health in Waite Park, MN seeks a Palliative Care Team Assistant to support our outpatient Palliative Care Clinic and Advance Care Planning program. This role emphasizes patient access, scheduling, and provider template management to create a welcoming, coordinated experience.

The ideal candidate is highly organized, detail oriented, and comfortable using Epic, managing complex schedules, and communicating with patients and families in a warm, professional manner to ensure timely care.

Qualifications

• 3 years medical secretary or clinic support experience required
• Previous scheduling and template building experience required
• Understanding of medical terminology

Responsibilities

• Manage and maintain provider schedules and Epic appointment templates
• Schedule new Palliative Care consultations and follow-up appointments
• Coordinate additional appointments and services (labs, imaging, etc.)
• Support patient check-in, verify demographics and insurance, collect co-pays
• Support Advance Care Planning Scheduler with new patient visits
• Assist patients and families with scheduling questions and coordinate across the system
• Provide general admin and operational support to outpatient Palliative Care
• Communicate with providers, nurses, patients, families, and other departments to ensure timely care
