Qualifications
- English language proficiency required per VA policy.
- Graduate of a school of professional nursing accredited by ACEN/CCNE.
- State licensure or eligibility to obtain licensure is required.
- Foreign nursing education requires credential evaluation where applicable.
Responsabilités
- Provide evidence-based, patient- and family-centered care within the PACT team.
- Document assessments and care plans in the medical record.
- Coordinate referrals to other disciplines and services as needed.
- Assist in development of the patient-driven care plan and ensure self-management goals are met.
- Maintain infection control precautions and safety standards.
