Job summary

Veterans Health Administration, Department of Veterans Affairs seeks a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) to provide comprehensive nursing care and care coordination for Veterans in the Primary Care Department.

The role requires English proficiency, valid RN licensure, and eligibility for state licensure as well as knowledge of care management and interdisciplinary collaboration. Work is on-site with a fixed schedule and VA total rewards.

Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE
• Current, full, active, and unrestricted RN registration in the United States
• English language proficiency for direct patient care
• Ability to obtain state licensure and transition from temporary to permanent appointment as required
• Foreign nursing education may require degree equivalency validation such as ICD

Responsibilities

• Assess, plan, implement, document, and evaluate care for a designated patient panel across the continuum of care
• Triage and apply a collaborative team approach to patient care problems
• Collaborate with patients to assess needs, goals and resources for discharge, recovery and wellness
• Maintain knowledge of multidisciplinary resources and refer patients to community services
• Use problem-solving methods to resolve system and process barriers to patient care
• Assist with navigation of health systems to facilitate patient outcomes
• Provide care respecting patient values, culture and beliefs
• Coordinate an interdisciplinary approach to meet physical, psycho-social, and environmental needs
• Work schedule: Mon-Fri 8:00 a.m. to 4:30 p.m. with intermittent as-needed hours
• Telework not available; this is not a virtual position
