Job summary

The Registered Nurse - Patient Aligned Care Team (PACT) Care Manager delivers and improves holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned.

As a VA RN in PACT, you will develop patient-driven care plans, coordinate services across teams, and educate patients and families to promote self-management and wellness, while maintaining licensure and advancing professional practice.

Qualifications

• English proficiency required for direct patient care.
• Graduate of a nursing program approved by ACEN or CCNE.
• Current, active RN license in US state or DC; license may be required for appointment.

Responsibilities

• Provide comprehensive nursing care to VA patients within the PACT.
• Develop patient-driven care plans with the care team.
• Educate patients and families on self-management and wellness.
• Collaborate with internal and community services to support care transitions.
• Maintain licensure and meet VA qualification standards.
