The Washington DC VA Medical Center is seeking a PACT Registered Nurse Care Manager to provide patient-centered, evidence-based nursing within a Teamlet of a PCP, RN Care Manager, and support staff. This role supports transitions and coordinated care within the VA system.

The position covers Nurse I–III progression with leadership, case management, health education, and coordination of care. Comprehensive VA benefits and a structured work environment are offered.

Competenze

• English proficiency required for direct patient care
• Graduate of nursing program approved by accrediting bodies
• Current RN license in a U.S. state or DC
• Foreign-educated nurses must have evaluation and licensure equivalence

Mansioni

• Provide evidence-based nursing care to Veterans within the PACT team
• Collaborate with interdisciplinary team to develop patient-centered care plans
• Coordinate patient care transitions between VA and non-VA services
• Participate in performance improvement and quality assurance activities
• Deliver care in accordance with VA policies and standards
