Job summary

Veterans Health Administration, Department of Veterans Affairs, is seeking an experienced Registered Nurse for the Patient Aligned Care Team (PACT) in Missouri. The RN will provide and oversee direct patient care in a primary care setting, supporting care coordination within the team while focusing on patient education and self-management.

Qualifications

• Graduate of a nursing program approved by ACEN/CCNE.
• Current, full, active, and unrestricted RN license in the U.S.
• Knowledge of ambulatory care and primary care practices is desirable.

Responsibilities

• Provide and supervise direct patient care in the PACT setting.
• Assess, plan, implement, and evaluate patient care using nursing process.
• Coordinate care education and self-management with patients across continuum.
• Administer medications and procedures per policies.
• Collaborate with interdisciplinary team to improve outcomes.
• Lead and participate in quality improvement within primary care.
