Responsibilities

• Functions in a shared workflow with a multidisciplinary PACT team.
• Provides leadership to the team including shared delegation of appropriate care and care processes.
• Provides comprehensive, collaborative same-day and continuity care to a panel of Veterans via face-to-face or telehealth modalities utilizing registries to enable population health management.
• Demonstrates excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members, and staff.
• Comfortable delivering patient-centered care in pain management, addiction treatment, mental health, women's health, chronic disease management, and geriatric care.
• Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
• Reviews available clinical and performance data with the team focusing on continuous improvement of critical team processes.
• Ensures the patient has same-day access for face-to-face and telephone care visits during regular clinic hours
