The US Department of Veterans Affairs is seeking a Licensed Practical Nurse to join a Patient Aligned Care Team (PACT). Under supervision of a Registered Nurse, you will perform practical nursing duties and support patient care in a patient-centered medical home model.

You will provide a full range of nursing care, administer medications as prescribed, educate patients and families, document care details, and collaborate with the RN/MD to promote safe and effective treatment.
