Responsibilities

• Coordinate end-to-end intake for prospective participants.
• Schedule home visits, center tours, assessments, and follow-ups.
• Maintain proactive communication with outreach, enrollment, and clinical teams.
• Educate participants on the PACE model and Habitat Health's services.
• Maintain accurate participant data in CRM/EMR and other systems.
• Support enrollment handoffs to the IDT and community events.
