Job summary

Midland Care Connection in Topeka, KS, seeks a Community Resource Specialist to help older adults stay independent at home through the PACE program. You will connect participants with healthcare services, housing, transportation, and public benefits, coordinating with an interdisciplinary team.

Candidates should have a high school diploma or higher, experience with aging services, knowledge of Medicaid/Medicare, strong communication, and the ability to advocate for participants.

Qualifications

• High school diploma or equivalent (some positions may require an associate or bachelor’s degree in human services, social work, healthcare, or a related field)
• Experience working with older adults, healthcare, social services, case management, or community outreach
• Knowledge of community resources, Medicaid, Medicare, and public assistance programs
• Strong communication and interpersonal skills
• Ability to coordinate services and work collaboratively with an interdisciplinary healthcare team
• Organizational and documentation skills for maintaining participant records
• Valid driver’s license and reliable transportation may be required for community visits
• Ability to maintain confidentiality and work with sensitive participant information
• Bilingual communication skills
• Familiarity with electronic medical records or healthcare documentation systems

Responsibilities

• Help older adults remain safely and independently in their homes by connecting them with healthcare services, financial assistance, and community resources
• Coordinate care with nurses, social workers, therapists, and the interdisciplinary PACE team to optimize outcomes
• Educate families about available services and maintain participant documentation
• Provide emotional support during challenging situations and advocate for participants’ needs
• Support access to housing, transportation, food assistance, utility support, and public benefits such as Medicaid and Medicare
