Job summary

Neighborhood Healthcare is seeking a PACE Center Operations Coordinator to support IDT processes and center operations onsite in Riverside County.

You will coordinate schedules, prepare agendas, track care plans, and ensure timely enrollee coordination while maintaining HIPAA and safety standards.

Qualifications

• High School Diploma/GED required; bachelor’s degree in business administration preferred.
• Current Basic Life Support (BLS) certification required; CPR and First Aid certifications required for all PACE employees.
• At least 2 years of administrative support experience required.

Responsibilities

• Coordinates Interdisciplinary Team processes, including scheduling meetings and calendars.
• Prepares IDT agendas, care planning materials, and documents decisions.
• Maintains IDT tracking tools for care plans and assessments.
• Tracks and follows up on IDT action items and timelines.
• Supports site operations and compliance documentation.
