Job summary

High Desert Pace Inc. is seeking a Care Plan Coordinator to oversee the full lifecycle of care planning for participants. You will lead an interdisciplinary team, facilitate care planning meetings, and drive quality participant services across departments.

The role requires strong analytical and leadership skills, with experience in health care settings serving the elderly. You will maintain care planning timelines, ensure policy adherence, and report to the Chief Clinical Officer.

Qualifications

• Minimum two years in a supervisory or clinical administrative role in health care delivery.
• Minimum one year experience with frail or elderly population.
• High School Diploma or GED; associate or bachelor degree preferred.

Responsibilities

• Lead care planning meetings and facilitate interdisciplinary team collaboration.
• Oversee care planning activities and ensure compliance with deadlines and policies.
• Coordinate with home care, nursing, social work, and other center staff to support participant needs.
• Develop and maintain care planning logs and generate bi-monthly compliance reports.
• Ensure care plans are signed and activated within policy timelines.
• Attend staff meetings and participate in HDP management activities.
