Job summary

Habitat Health is seeking an Enrollment RN to lead clinical aspects of the enrollment process. You will conduct in-home nursing and safety assessments, coordinate with the IDT, and ensure compliant documentation in the EHR system.

You will educate participants and caregivers, build trust, and help navigate PACE enrollment while traveling within the service area. A California RN license and BLS are required.

Qualifications

• Associate's degree in Nursing or equivalent required.
• Active, unrestricted RN license in California.
• Minimum of 1 year nursing with older adults or complex medical needs.
• Ability to perform clinical and home safety assessments with clinical judgment.
• Strong organizational skills and attention to detail.
• Clear communication and cross-functional collaboration.
• Experience with EHRs and enrollment documentation systems.
• Valid driver’s license and ability to travel within the service area.
• CPR/BLS certification or ability to obtain within 30 days.
• Meets health requirements including TB and immunizations.

Responsibilities

• Partners with IDT to support a coordinated enrollment experience.
• Conducts nursing, home safety, and eligibility assessments at homes.
• Completes NFLOC assessments per DHCS requirements.
• Presents clinical findings at enrollment eligibility meetings.
• Performs home safety evaluations for prospects.
• Educates participants and caregivers, building trust.
• Maintains accurate clinical documentation in EHR.
• Keeps knowledge of PACE eligibility and enrollment processes.
• Supports outreach and community engagement as needed.
• Contributes to workflow and operational improvements.
• Performs other related duties as assigned.
