Job summary

MorseLife Health System, Inc. seeks a PACE Community Liaison to manage intake and enrollment for new participants and to provide information about the program and the community.

You will build relationships with referral sources and coordinate enrollment meetings, supporting families and caregivers through the process. The role requires 3–5 years in social work with frail elderly care, excellent communication, and the ability to handle multiple priorities.

Qualifications

• Bachelor's degree or above, preferably in Social Work. Master’s degree preferred.
• 3-5 years of Social Work experience with frail elderly and families required.
• Strong communication skills and ability to manage multiple priorities.

Responsibilities

• Receives and generates referrals from agency staff and community sources.
• Establish and maintain working relationships with prospects and referral sources.
• Develop and distribute program materials to inform the community and prospects.
• Serve as a liaison between prospects, caregivers, and community referral sources.
• Coordinate enrollment meetings and answer non-clinical questions regarding patient rights and program rules.
