Job summary

Volunteers of America, Senior Community Care of Colorado PACE in Montrose, CO is seeking an Enrollment Specialist to coordinate admissions and referrals for the PACE program. This paid role supports outreach, enrollment, and community partnerships to help older adults access comprehensive care.

The role requires a bachelor’s degree in humanities/health and human services (or equivalent with CNA/LPN), plus 2 years marketing/admissions experience and 1 year with frail/elderly populations.

Qualifications

• Bachelor's degree in humanities/health and human services or equivalent experience with CNA or LPN certification.
• Two years of marketing experience with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks.
• Have a minimum of one year of experience working with the frail or elderly population.
• Must have a valid driver’s license and transportation within 100 miles daily.

Responsibilities

• Gathers admission information, coordinates intake process, and provides information and referrals to community agencies.
• Promotes the Senior CommUnity Care Program to potential participants and community agencies.
• Assesses potential participants in home, nursing homes, hospitals or other settings as part of the intake process.
• Assists with Medicaid eligibility process for participants.
• Schedules enrollment meetings with potential enrollees.
• Reviews Medicaid eligibility, monitors re-certification and facilitates Medicaid applications.
• Initiates enrollment activities to meet census goals and supports marketing department objectives.
• Performs related duties as required.
