Job summary

PACE Place seeks an Intake & Outreach Coordinator to support enrollment growth by developing referral sources, generating qualified referrals, and enrolling 3–5 new participants monthly. You will serve as a primary liaison to referral partners and coordinate intake with the IDT.

Under the Outreach & Enrollment Manager, you will conduct compliant educational outreach, maintain CRM data, track pipeline, and ensure timely handoffs to Medicaid specialists.

Qualifications

• Fluent English required; secondary language a benefit.
• Experience with Medicare/Medicaid waiver program preferred.
• Experience with service providers in community-based settings.
• Strong communication and interpersonal skills.
• Must have valid Florida driver’s license and reliable transportation.

Responsibilities

• Maintain referral source relationships across clinical and community settings.
• Serve as primary day-to-day contact for referral sources with consistent communication and education.
• Manage referral pipeline, prospecting new relationships and following up with inactive sources.
• Coordinate intake and enrollment processes, including eligibility screening and handoffs to Medicaid Specialists.
• Enter and maintain referral, intake, and enrollment data in CRM and tracking systems.
