Job summary

PACE Partners of Northeast Florida in Jacksonville, FL is seeking an Intake & Outreach Coordinator to support referral development, enrollment of 3–5 participants per month, and liaison with clinical and community partners.

You will conduct compliant outreach, manage intake and enrollment processes, and collaborate with Marketing, Medicaid Specialists, and the IDT to ensure timely referrals and accurate CRM data.

Qualifications

• Fluent English; second language a benefit.
• Experience with Medicare/Medicaid waiver program preferred.
• Experience in human service care delivery system.
• Experience with community-based providers in aging services.
• Knowledge of Medicare and Medicaid programs.
• Strong written and verbal communication and presentation skills.
• Ability to work autonomously and as part of a team.

Responsibilities

• Develop and nurture referral source relationships across clinical and community settings.
• Actively manage a referral pipeline, educate partners, and re-engage inactive sources.
• Coordinate intake and enrollment activities with Medicaid Specialists.
• Maintain CRM data, monitor pipeline activity, and report updates.
• Support outreach events and educational sessions for program awareness.
