Job summary

North East Medical Services (NEMS) is seeking an Enrollment Specialist to orient potential participants and referral sources about the PACE program and to intake new enrollments. The role requires independent information gathering, scheduling assessments, and documenting care needs, while presenting NEMS PACE to families and community partners.

You will conduct center tours, build relationships with medical providers and organizations, and guide participants through enrollment and the LOC

Qualifications

• Experience in sales or community outreach is required.
• Fluency in English and strong communication skills are essential.
• Comfort presenting to groups and partner organizations is expected.

Responsibilities

• Serve as the main contact between the PACE team and potential participants or referral sources.
• Provide clear information about PACE and enroll eligible participants.
• Gather information, conduct screenings, and schedule assessments for enrollment.
• Educate patients and caregivers about services and the enrollment process.
• Develop relationships with medical providers and community partners to support referrals.
• Assist and oversee enrollment through the LOC process and document care needs.
