Job summary

FreedomCare is seeking a Case Resolution Specialist I in Pennsylvania to partner with patients, caregivers, and health plans to resume or start homecare services. This field-based role requires 2-3 days a week in our Pittsburgh office and focuses on intake, enrollment, and one-point contact for the customer experience.

You will manage high volumes of calls, document cases in Salesforce, coordinate authorizations, and guide families through Medicaid and health plan processes.

Qualifications

• Experience managing a high volume of inbound and outbound calls.
• Excellent empathetic communication and active listening skills.
• Proficient note-taking in a CRM and documenting case statuses.
• Ability to guide patients and caregivers through enrollment processes.

Responsibilities

• Provide high-level customer service through one-call resolution with a helpful demeanor.
• Serve as the primary contact for patients and caregivers restoring services.
• Navigate systems to determine eligibility and obtain authorizations.
• Maintain detailed patient and caregiver case notes in Salesforce.
• Meet daily, weekly, and monthly metrics for enrollment and call handling.
• Assist patients and caregivers with completing the process to restore services.
