FreedomCare is seeking a Field Care Specialist I to support patients in Pennsylvania. This role requires frequent travel across the Philadelphia area and involves direct contact with patients, caregivers and care coordinators to ensure timely home visits and compliant documentation.

The ideal candidate will have 1+ year field experience, excellent communication, empathy, and the ability to work independently with transportation.

Qualifications

• 1+ years of field-based experience.
• Ability to travel daily within the Philadelphia PA area.
• Strong written and verbal communication skills.
• Must have your own reliable transportation.
• Proficient in data input and documentation.

Responsibilities

• Manage an assigned patient's caseload.
• Direct point-of-contact for enrolled member questions, inquiries or concerns.
• Collaborate with Care Coordinators in scheduling home visits.
• Conduct home visits and complete necessary documentation.
• Review results of home visits and create follow-up steps.
