Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing preferred.
• Compassionate, team-oriented with eagerness to learn.

Responsibilities

• Coordinate resident admission processes, including screening, assessment, and care needs.
• Ensure care is delivered per resident care plans.
• Administer medications and treatments per physician orders.
• Coordinate care with outside providers including resident physicians.
• Document care provided and report observations, including significant changes.
• Maintain knowledge of state regulations, policies, and procedures.
