Responsibilities include coordinating admissions screening and care plans, administering medications per physician orders, reporting observations, and maintaining knowledge of state regulations.

Qualifications

• Knowledge of state regulations, policies, and procedures applicable to nursing.
• Graduate from an accredited school of practical nursing and valid nursing license.
• Licensed LPN or RN with state registration and ongoing professional practice.
• Experience in geriatric assessment and nursing is preferred.

Responsibilities

• Coordinate resident admission through screening and assessment and care needs.
• Ensure care is delivered per resident care plans and physician orders.
• Administer medications and treatments according to orders.
• Coordinate care with outside providers including residents' physicians.
• Document care provided and resident observations, noting significant changes.
• Maintain knowledge of state regulations, policies, and procedures.
