Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse to support the health and well-being of residents through clinical monitoring and collaboration with care teams. The role includes assisting with admissions, care planning, medication administration, and coordination with external providers to uphold regulatory standards.

Ideal candidates hold an LPN/RN license with general knowledge of geriatric care, and a compassionate, team-oriented approach.

Qualifications

Graduate from an accredited school of practical nursing.
Current nursing license and state registration (LPN or RN).
Three years’ experience in assessment and geriatric nursing, preferred.
Working knowledge of state regulations, policies, and procedures.
Compassion and eagerness to learn.

Responsibilities

Assist in coordination of the resident admission process including screening, assessment, and care needs.
Ensure care is delivered as indicated in resident care plans.
Medication and treatment administration and ensure medications are administered per physicians’ orders.
Coordinate care and services with outside providers including resident physicians.
Document and report care provided and resident observations including unusual or significant changes.
Maintain a working knowledge of state regulations, policies, and procedures.
